The WV Select Care Management Services Program is an undertaking of the Choice Care at Home, a social services agency that has operated in West Virginia since 1980. Choice Care operates several programs designed to assist various populations in developing and maintaining an independent lifestyle. Of these programs, the case management is the largest. As one of the largest case management agencies in the state of West Virginia, Choice is committed to providing services for all the qualified residents of the state either on a private pay basis or through participation in state or federally-sponsored programs.

Affordable Independence

The Care Management Services Program is designed to act as an affordable long-term healthcare alternative for West Virginia residents who would normally be placed in a nursing home or to provide temporary assistance for those recuperating from an illness, injury, or surgery. Clients remain in their own homes, supported by a package of services that is custom designed to assist them in maintaining their independence for as long as possible.

Services are contracted, coordinated, monitored, and managed by an experienced professional care management team consisting of West Virginia Licensed Social Workers, Registered Nurses, and knowledgeable administrative and support personnel.

Eligibility

Admission into WV Select’s Care Management Services Program is open to any individual evidencing a need for long- or short-term care and meeting basic medical and financial criteria.

Services

Homemaker/Personal Care Services

Assistance with shopping, meal preparation and cleanup, laundry, housekeeping, and related chores, as well as personal care assistance with activities of daily living.

Custodial Care

Consisting of companionship, client monitoring/supervision, light meal preparation, and clean-up.

Private Duty RN Services

Medication Set-Up/Administration

All clients will have their care managed by a licensed RN and/or Social Worker as appropriate and access to 24/7 on-call services.

WV Select Care Management Services is a program recognizing an individual’s inherent right to exercise personal choice in long-term care decisions. Designed to act as an alternative to nursing home care, WV Select is committed to providing an extensive range of top-quality services that will enhance both the length and quality of life in the familiar security of one’s own home. WV Select can implement services virtually upon request, and every effort has been made to keep costs within the reach of those who have planned financially for their future.

The overall goal of WV Select is to provide the client, his or her family, or other responsible parties with all services necessary to maintain a quality standard of independent living with total freedom from management, coordination, and contracting responsibilities.

Services provided under this program are typically paid for by the client, the client’s family, trust funds, insurance companies, etc. From basic assistance to highly specialized care, WV Select is “the right service at the right time.”
